Addon only works SOMEtimes?

I like the addon, but I rarely see the actual alerts come up. Sometimes they work, but not very often. Is there a reason for this? Also, it would be REALLY helpful to have some type of visual indicator for in/out of combat. I'd like to know when I've drawn agro on something.

Actually the unit frames are hidden while not in combat? or will you add this feature later? (seems like an obsession for me to have my UI nearly as empty as i can :P) 2nd'd... a simple visual indicator if IN/OUT of combat would be very useful, thanks.
