Check your enabled addons which is about quickslots or UI tewaks (like AUI, Lui, Bandits UI, etc.).
Or use the addon "Addon Selector", save your current enabled addons as a profile/pack.
Disable the lower half of it (you can slick the 1st addon and then SHIFT+left click any other addont o mass mark/demark the addons...

I found it. The ZOs provisionier settings got 1 entry named showProvisionerSkillLevel which controls if the skill header will be shown or not.
This is defined in 2 ZOs settings, ZO_Provisioner.PROVISIONING_SETTINGS (visible) and ZO_Provisioner.EMBEDDED_SETTINGS (hidden).

Update: Seems it's that deep into pChat that we are not able to fix it without breaking other stuff.
Maybe one day, when newer libs were born which handle the chat formatting, and history accross addons.
