This addon is the revive of the original "Simple Combat Alerts" by Dio:

Needed dependencies (not included! Please install them just like an AddOn):
LibAddonMenu-2.0 version r28 or newer
LibMediaProvider 1.0 version r13 or newer

Simple Combat Alerts Revived replaces the default Active Combat Tips with less cluttered and customizable tips.
Attention: Combat tips will be automatically enabled if you use this addon (at login/zone change/reloadui).
It is not able to deactivate the combat hints automatically if you disable this addon so you need to keep track of this yourself please!


Alerts are prioritized in the following order:
Block -> Off Balance (Shows after blocking or interrupting) -> Interrupt -> Dodge


Features
Customize the text, font, size, style, and color of each alert.
Unlock and position the alert anywhere on the screen.

Difference to the original addon:
-Using up2date settings menu LibAddonMenu-2.0
-Changed min and max values of the alert's font size
-Fixed errors
-Automatically enabled automatic combat hints if this addon is enabled
-Some speed ups at e.g. alert detection

-Fixed dependencies to list and need: LibMediaProvider-1.0
-Added support for addon "Synced Account settings" which syncs the account settings of addons 5 seconds after the player is ready ingame / after reloadui / after zone switch e.g.
SCAR will try to change the combat hints to enabled after 7 seconds again if this addon is enabled.

Fixed initial position of the alert label on the screen (was not shown on first preview click and was shown at the most top left corner)

The Chaurus centipede mobs in Blackreach. Block or dodge alerts don't show up with this addon for them, but do with normal combat hits turned on. Just something I noticed while farming a couple of days ago.
Okay, thanks for that extra info. Would be good to have you posted that as your first post already. Makes things clear from the beginning as we cannot read your mind

Weird. The addon uses the game's mechanics that fire as the standard combat hints are shown, and just shows other info than. So it should always work, no matter which mob or whatever stuff raises the hint. The hint itsself is firing an event, where this addon listens to, and reacts afterwards. Maybe the event is bugged somehow for those mobs.

Hadn't have time to play for a week now, I'll test this some day.
