Check the addon's ZIP archive. Are your files in a subfolder (foldername = addon name in most of the cases) or are the files located directly in the zip archive?
It has to be in a subfolder inside the archive and the manifest file must be correct too.
What is the name of the addon that does not show up anymore?
I guess it is ChampionUI. The ZIP file only contains 3 files, but no folder.
Put the 3 files into a folder called like your addon, e.g. ChampionUI, and then zip the folder including the 3 files in it.
Reupload it (you can check the checkbox to ignore version number to just replace the filename, and uncheck the checkbox to archive the current file as it is broken).
