I think it occurs more frequently when the player population in the area is higher, because I wasn't experiencing it until TU. I have never experienced this issue and I have played since beta.
As I wrote earlier, then I suspect a bugged client. I know it's a pain but I think a full uninstall/reinstall is the way to go.
