multiple UI bugs

When I enter the game, these three windows appear with error:

Could someone help me to resolve this?

FCMQT is not a maintained addon, you can use my Vestige's Epic Quest (which is based on FCMQT) if it fits your needs or wait eons (or sooner) for a messiah to release an updated version of FCMQT.
